Babies are often evaluated based on their weight gain, which can lead to the assumption that they are feeding well at the breast. When a breastfeeding parent reports no significant pain or physical damage, it is sometimes assumed that the baby has a deep latch and is feeding effectively. However, pediatricians typically do not evaluate the parent, and they may overlook potential oral restrictions, such as a tongue, lip, or cheek tie, especially if weight gain is on track.
It’s also common for pediatricians to assess a baby's sucking ability by placing a gloved finger in the baby’s mouth to observe their sucking response. From my experience, babies are born with a natural "suck reflex," so they will instinctively suck on a finger placed in their mouth. However, the sensation and length of a finger are quite different from a nipple, and other factors like short nipples, oral restrictions, or engorgement can make it difficult to gauge feeding effectiveness based on finger-sucking alone. Lactation consultants, on the other hand, are trained to assess a wide range of factors impacting breastfeeding, including tongue mobility and other issues that may affect feeding beyond just the baby’s ability to suck on a finger.
Your baby should always stay up to date with pediatrician visits, but when it comes to infant feeding, lactation consultants are the experts. We take into consideration the “whole picture,” meaning we aren’t only looking at any symptoms the baby might have, we are also evaluating and assessing the parent to insure there aren’t other underlying problems that can affect feeding or weight gain.

I just wanted to let you know how much your advice is helping already! Before each feeding I've been doing the massages. He's been eating so much better & very content after just 20 minutes of nursing. He's napping so much better in between feedings & i noticed he's not so stiff.
